Your child's therapist will need the Intake Form at the first appointment. When you open the form, you can complete it on your computer and print it or print the blank form to complete by hand. The completed form might not be able to be saved to your hard drive, so be sure to have plenty of time to complete the form once you start. Bring it with you to the first appointment. Some forms give the option to email back to us. We are still working on getting this feature working properly. Please print the forms and bring them with you for now. If you would like us to have information from your child's teacher, please print out the appropriate form, give it the teacher to complete. The teacher can return the form to you or fax it back to us at 303-649-9008.
